Open Enrollment - Affordable Care Act

Learn about changes taking effect in 2015, meet with trained experts, and even get help enrolling to avoid lapses in coverage. Formal presentation begins at 6 PM. This program will be repeated on Wednesday, January 7, 2015 at 5 pm.

*Open enrollment for 2015 ACA coverage is November 15, 2014 - February 15, 2015.*

Presented by certified application counselors from the Community Health Centers of Greater Dayton. No reservations are necessary.

NOTE: To receive personal enrollment assistance, please bring the following: 1) personal email address & password; 2) social security numbers of all household members; 3) income verification for all household members (paystubs or tax information from last year); and 4) a current insurance card (if any) including amount of monthly insurance premiums.
